If you have an on-the-job injury or occupational disease...

Follow these simple steps:

Your MANAGED CARE ORGANIZATION (MCO) is SIERRA HEALTH-CARE OPTIONS (Sierra at Work).

Report the injury or occupational disease to your supervising administrator immediately! Fill out a “Notice of Injury or Occupational Disease” Form C-1.If treatment is required for non-emergency conditions, see a medical provider at any of the CCSD-approved SOUTHWEST MEDICAL ASSOCIATES URGENT CARE CENTERS or PREFERRED MEDICAL CLINICS listed below. YOU DO NOT NEED TO MAKE AN APPOINTNMENT FOR YOUR INITIAL VISIT.

For life-threatening conditions requiring immediate treatment, call 911 or seek treatment at the nearest hospital or emergency room. Notify your supervising administrator and complete the required forms as soon as possible thereafter.

Examples of life-threatening conditions:

  • Loss of consciousness
  • Spinal injuries
  • Major burns
  • Poisoning
  • Shock
  • Severe chest pain
  • Major burns

To have a prescription for your on-the-job injury or occupational disease filled at no cost to you, you must select an EXPRESS SCRIPTS participating pharmacy. Most pharmacies are on the approved Express Scripts list EXCEPT Walgreens and CVS. For more information or additional locations, contact Express Scripts Customer Service at (800) 677-8879.

Follow-up visits including appointments with a specialist, if required, will be coordinated by an MCO Nurse Case Manager, (702) 838-8290.
